I left a 100% tip once. The food was extremely good, the waiter super friendly, good atmosphere and very cheap. We ate together for 25 euro, including drinks. So we gave 50 euro. This was 20 years ago or something, so prices were different, but it was too cheap for the quality we got. It was worth the look on the face of the waiter alone already.

Yes, but in the early stages you normally cure completely. If you have it for a year or so before getting diagnosed, then there's much less chance on complete recovery. I had the early stage and I think I don't have any lingering effects.
