Yep. He has trouble breathing because of his malformed snout. Putting his foot in his mouth helps him breath. Not sure how he discovered it.

suspension of disbelief. Granted, this is RPG not sandbox. But player choice is a core theme among RPGs. Sandboxes facilitate choice.
