This happens when you use the wrong size bit for the screw. Many people don't know there are several sizes of Phillips screws and bits. They need to match.

I grew up in Phoenix. Believe it or not traffic used to be much worse 30+ years ago.
When I was very little a ladybug landed on my arm. I was quite upset. My mom said "Don't worry, it's just a ladybug". I said to her "WELL, LET THE LADY HAVE IT!"
