Plus you can see the top right side of his head has been expanded outward to cover the spot of the other persons head.
pretty sure it's photoshopped. The hands and neck look to be of someone who is younger and has a higher body fat percentage.

if a women ends up with two copies, she will have 4 cones in her eyes instead of 3 making it possible to see colors beyond violet. 3/3
