Why aren’t US citizens out on the streets? I’ll try to answer: A tiny % of the US owns most of its wealth and everyone is almost broke. Most people in the US are just working and sleeping. That’s why the people aren’t marching, everyone’s just surviving. It’s not bad enough enough for a revolution because we can still eat and not enough to use what free time we have to know what’s going on.

If it were up to me we’d support Ukraine unequivocally. You can’t have peace with Putin. Signing a piece of paper that says “treaty” means nothing. Too many people have died. You can’t just snap your fingers like Trump thinks you can… “make a deal” and all is well. Even after this is finally “over” there’s going to be partisan warfare for generations. Until russia decides to join the rest of the world’s republics. Russia would be an asset to humanity if they got rid of their greedy dictator.

I was born and raised, currently work and live in Minneapolis and the surrounding suburbs. Minnesota is always in the top 5 with election turnout. Republicans will get crushed here in November. Trump will no doubt try to stop democratic strongholds from voting. He’ll use ice everywhere to simply be an intimidating presence at polls under the guise of election integrity. I can see it as clearly as I can see how corrupt this country has presently become.

I’m not defending our system. It’s a gross system that works well for a few and ok and quite badly for a lot. It’s a doubled edged sword. If you have money. Which most people don’t. Or if you have a really good healthcare plan. Or are super poor and get free healthcare, you do have access to the best acute care system in the world. But if you’re middle class with just one chronic condition or unexpected illness with a bad health plan you’re getting squeezed hard…

Even for snacks and “our precious loved one needs to eat now!” We have to waste time explaining a lot to our ignorant people. Plus patients that roll into the er drunk. Say they’re suicidal and have a plan to do it, then they get moved to the front of the line, get a bed and just sleep, watch tv, eat, make demands, don’t want to leave. There’s a whole lot of reasons why that’s the case, we also can’t refuse anyone by law. Should we start doing that? People come in to fill meds too…

I work in an er and that’s not even a long wait. It’s an ecosystem. Has to do with physical beds available. Acuity of patients. Availability of housekeepers, nurses, providers to take care of patients and discharges on the floor and a lot of other things. It also has to do with the fact that people are sicker than ever. Everyday people get sicker. I often work without a break for 12 hours straight not sitting down maybe more than 5 times all to chart. Patients also demand service now…

I don’t think there’s a perfect system. Single-payer models do some things very well, and the U.S. system does other things very well. The problem is that the U.S. starts from a very different place, which makes simple comparisons or quick fixes unrealistic.

Covering a population with this level of chronic illness would be extremely expensive, especially early on.

There’s also the workforce side. U.S. healthcare workers are paid more than in most single-payer systems, and any major change would likely mean pay cuts and higher patient loads. That has real consequences for staffing and burnout.

We’re very good at handling complex, high-acuity problems when time matters.

Part of the reason we have to be that good is because we don’t do a great job with prevention. Our population is sicker overall, largely due to lifestyle, diet, and chronic disease. That creates a huge amount of demand on hospitals and emergency care.

Because of that, I’m cautious about assuming a single-payer system would work the same way here as it does in Europe.

That’s very different from most of the U.S. system, where productivity matters and providers are paid more for doing more.

Many of the Canadian patients I’ve taken care of have said the same thing about their system especially when it comes to waiting for non-urgent care or specialists.

Where the U.S. system really shines is in emergencies. If someone comes into my ER with sepsis, stroke, or major trauma, we can move incredibly fast.

I’ve never lived in a single-payer system, but I have worked in one the VA. From that experience, the wait times are real. A lot of it comes down to incentives and basic human behavior.

At the VA, providers are salaried. A doctor gets paid the same whether they see five patients in a shift or twenty. That setup has upsides, but it also affects how quickly patients move through the system…
