It's called a dermoid cyst. It can grow portions of body parts, pretty wicked.
I think you can see it because how the mouth moves when they talk, it's a small difference but we humans tend to notice small differences.

I dunno how correct they are though, very little-to-no fire and the smoke is white instead of black. Seems like left-over chems got mixed ->
