It's probably only being used here because of the increase in diameter from the pipe to the much larger shower head...
It's not *adding* anything. It minimizes pressure loss by making sure that the incoming water doesn't run into the outgoing water....
Angled spinner blades help ensure all incoming water is flowing quickly, smoothly, & in a proper direction until it has exited the nozzles
