If it's metastisized to the bones then it's stage 4 and terminal.
Most clinics and hospitals are privately owned and many by insurance companies, so the insurance company/proprieter usually gets the deductible in that case. Doctor usually doesn't get shit.
I think that's an overly charitable summary tbh. Republicans believe that if you need the charity to avoid going hungry, you aren't really worth feeding anyways. Claiming that people are cheating it is just a dogwhistle to save face.
