It sucks and is a guessing game every year if you’re lucky enough to have a couple plan options to choose from with various deductibles.
The cost of medications typically don’t go toward deductible unfortunately. And yes, deductibles reset every year.
So maybe I’m biased, but IMO the term doesn’t really place the blame appropriately
I work in healthcare, and we can’t post fees for every procedure or visit type because even we don’t know what will be covered by dif plans

VSP makes silly rules like the optometrist has to own 80% of optical... aka not what happens in a hospital or multidisciplinary setting

And many insurances will give you one for free each pregnancy.
