During my training it was not uncommon for me to work 13 days in a row, closer to 14h/d by the time signout and scut work was done. This is not unique in medicine and some residents have it works than that.
12h/d x6d/wk without breaks for bodily functions like eating and going to toilet? You just described the “improved” work rules for medical residents in the US. Not all hospitals and residency programs abide by these, many still don’t have caps on work hours.

Wouldn’t it constitute insurance fraud to “pretend” to have a medical problem?

As an obgyn, I can categorically say, you are wrong. I care very deeply about women. Otherwise I would not have put myself through the hell that is becoming (and sometimes being) an obgyn.
