Sith don't all start as Sith. There were presumably no Sith after episode 6, but thanks to Luke training more Jedi, we eventually get Kylo.

That's no longer canon. The new canon is that Sith lightsabers are red because dark side users "bleed" regular kyber crystals and turn them.
The Jedi hunt natural crystals as part of their training. The different colors usually signify their role in three Jedi order.

It's only during the creation of the lightsaber. Nothing would stop Darth Vader from using Luke's green lightsaber for example.
That said a Jedi could still purify a bled crystal leaving the color white (Only Ahsoka has done this so far)

Right now yellow is only seen on Jedi Temple guards, and there is no canon orange one yet.
The way Sith create their sabers is by bleeding the crystal, which causes the crystal to bleed, hence the red color.
We don't actually know where Ben got his crystal, it could very well be the one from Darth Vader's saber.
I thought kylo wasn't fully on the dark side yet. From what I can tell his shouldn't have been fully red yet. I've seen it once only though
