#41 you've just got to think outside the box a bit as a DM is all. Once started a campaign where I had everybody make two characters. The one set were all meant to be either in, or heading to, this small village on the border or two rival kingdoms. I let the players do some individual tasks around town before, BOOM, magical nuke, laying waste to village and PC alike. Scene immediately cuts to their second characters, who somehow survived the blast only to then be rounded up in prison transports.
