Combination of both, really. Ones that large are designed to be used horizontally only. This cut looked like it was going ok otherwise but he shifted causing the blade to grab and that's why it shot out. Someone with better upper body strength may have been able to hold onto it and at least keep it from bouncing around the whole room.
