Truck driver here. Maximum height for trucks without special permits is generally 13' 6". One of the Boston tunnels is 13' 8", one is 12' 9"

The Sumner and Callahan tunnels were opened in 1934 and 1961 respectively, when trucks were smaller, so 12'6" max height wasn't an issue.
