#19 That looks like the Pathfinder artwork. Pathfinder is published by Paizo; I suspect someone threw on the Wizards copywriter by mistake.

The first few Pathfinder books were sourcebooks for 3.5. Also artists aren't tied to a single publisher. But yeah looks a lot like early PF.

Yup. That's more or less where Adrian Carmack copied the cacodemon from. Iirc, the monster is meant to be an astral dreadnought.
