When an animal is neutered they remove the testes but leave the scrotum intact. What you're seeing is both fuzzy penis sheath and scrotum.

They don't take the sack, only the balls. Cat gets to keep his sack. It's heathier that way.

There's no health benefit at all to leaving it. There is however several downsides to removing them at all, but as a rule vets ignore them.
