He is too powerful for the Marvel universe, so they made it so him remembering his full strength adds a villain equally as powerful.

So they made him a bit mentally unstable to make up for it, sticking him in a monastery with Wong to super-charge Strange is another way -->

The torment, IIRC, was physical. And he's capable of withstanding incredible pain, he barely feels it.
