As near as I can tell, whatever it is is heavier than he expected it to be, so it has him pinned against the rail, it's too heavy for him to lift, so he can't get it back up on the steel, and he can't get the lift to go down because of a safety interlock.

Okay, I did some looking. I found a longer video that shows more at the beginning, and found some news stories. The thing he is taking down is a patio furniture set that weighs ~120 lbs or so. He was not supposed to be using an order picker to get it down, but other employees told him to do it anyway and can be heard heckling him in the longer video before he gets pinned and starts yelling. That's a completely different machine. Order picker is drivable and has a flat base outside of where you stand that you put the large, heavy stuff on. THAT lift (simply "blue lift") is push-only, only has the part where you stand, and is supposed to be used for small items. If he didn't know better, that's on him. They preached safety at Lowe's when I worked there. Should have known better than to get that down that way.

Impossible to know without finding an article or seeing how he came down. The blast upward probably wouldn't - the force of the lid flipping looks like it hit his arms and legs, and the rest was pressure, so dangerous, but relatively distributed.

Saw other folks claim before that spotter told lifter he shouldn't be lifting that much multiple times because he had failed multiple times and he wouldn't help even if lifter got in trouble. Lifter then proceeded to lift anyways, fail, and get scolded by spotter.

Having worked at Lowe’s I will say that for the blue lift a spotter is not required and so likely he was just another associate in the area. The biggest mistake was using that lift for this, they should have used the platform lift(which does require a spotter). Further, that has sensors that turn it off if someone gets close and it has to be reset to continue so in his panic he likely forgot that and the second guy shouldn’t have approached until it was all the way down.
